Two Victorian care organisations were moving to new care management platforms under compressed timelines.

One organisation was implementing The Lookout Way across home care. The other was already well into an AlayaCare migration and facing delivery risk. The platforms were different, the starting points were different, and the delivery conditions were different.

The same pressure sat underneath both programmes.

Platform change in care settings reaches far beyond system configuration. It changes how teams understand client information, complete daily work, manage handovers, follow process, make decisions and maintain confidence during transition. When those operating questions are left too late, the risk rarely appears as a technical failure. It appears as confusion at go-live, inconsistent adoption, unresolved ownership and months of post-implementation debt.

Both organisations needed more than platform delivery support. They needed implementation leadership that could connect the system, the operating model and the people expected to work differently from day one.

Solution

Alyve embedded experienced implementation leadership into both programmes, working close to the project detail and the operational teams that would need to adopt the change.

For The Lookout Way implementation, Alyve placed an Implementation Lead inside the project team and an Operating Model SME directly with the General Manager of Home Care. The work ran across six months and made the future operating model tangible before go-live. Day 1 operating models were developed for each function, reflecting the different levels of change across teams. Role-based personas and journey maps connected training to the way people actually worked. Four regional showcases gave staff early visibility of the platform and created a more confident path into transition.

For the AlayaCare implementation, Alyve entered mid-programme and moved quickly. The team assessed the delivery exposure, tightened governance, supported the vendor relationship and led the operational readiness work required to protect the go-live. The focus was practical and connective: linking technical delivery to the operating consequences that leaders, managers and frontline teams would experience from day one.

Across both programmes, Alyve created the missing bridge between the platform and the organisation. The work gave leaders clearer control, gave teams a more practical view of the change, and brought operational readiness into the programme while there was still time to shape the result.

Outcome

Both care organisations reached their planned go-live dates under significant regulatory, vendor and operational pressure.

The Lookout Way transition landed strongly. Senior leaders cited the programme at executive level as an example of best-practice delivery, and the Group CEO described the transition as smooth, with staff engaging positively from day one.

The AlayaCare implementation went live on time, with zero critical defects and 98% of core data successfully migrated. AlayaCare noted the turnaround to go-live was among the fastest they had seen.

The value for both clients was confidence through the hardest part of platform change. Leaders had stronger governance. Teams had clearer ownership. Frontline users had earlier exposure to the system and a more practical understanding of how work would change.
